Clinical services will be charged to you based upon the following fee schedule. Session length is determined at the clinician’s discretion. Telephone conferences and court related clinical services may be invoiced at the appropriate regular fee and are not covered by insurance.

Please be advised that while we may accept payment from many different insurance carriers, we may not be in-network with all of them. As a courtesy, we will bill your in-network insurance company. At this time, we are in-network with Blue Cross Blue Shield PPO and Loyola HMO.

There are many insurance companies and policies. Your clinician reserves the right to determine whether the office will bill your out-of-network insurance company. If we are unable to bill your insurance company, our office will provide you with a receipt of services to submit the claim on your own. At this time, CBH is unable to provide services for Medicare, Medicaid, All Kids and HMO policy holders.

Insurance coverage is a contract between the patient and their insurance carrier. We will assist you in maximizing your insurance benefits by verifying your benefits before the initial date of service; however, please be advised that this is not a guarantee of payment. It is your responsibility to verify and understand the benefits of your insurance plan.

Please verify your insurance coverage by asking the following questions:

      1. Do I have mental health insurance benefits?
      2. What is my deductible and has it been met?
      3. What is the coverage amount per session?
      4. Is pre-approval or approval from my primary care physician required before attending?
      5. Are there any limits on the number of sessions attended per year?

Cancellation Policy

If you do not attend your scheduled therapy appointment or if you have not notified us at least 24 hours in advance of a cancellation, you will be required to pay the full cost of the session.
